Remote patient monitoring is a digital health innovation developed to meet challenges such as an aging population and the growing burden of chronic illness. Patients with chronic conditions are often managed by both primary and secondary care, hence collaboration and coordination are recommended in remote patient monitoring interventions. This study aims to describe and get insight in the coordination processes when municipalities in primary care and hospital in secondary care work together during implementation of remote patient monitoring (RPM) service.
